Change the sound output settings on Mac
Plug in both ends of the HDMI without the laptop on. Turn the external monitor on and set to HDMI screen. Boot up your laptop from a shutdown position not restart. Your laptop will now recognize the external monitor and will acknowledge its presence. Right-click the speaker icon on the task bar and choose View Volume Controls.

My laptop only has a "IDT audio codec", which has no way to change settings. It does not have Realtek. Try going to device manager when screen is plugged in and with nvidia drivers updated and right-click on sound, video and game controllers and click on check for new hardware.
